The mechanisms by which topical metronidazole reduces the lesions and erythema that accompany acne rosacea are still not fully understood. Despite the antimicrobial effects of metronidazole recognized, there is no evidence that the elimination of bacteria or parasites and accommodated in the skin is directly responsible for the beneficial effect of medication on this form of acne.
The results of in vitro and in vivo showed that metronidazole has an anti-inflammatory aimed directly neutrophil chemotaxis and cell-mediated immunity. We also noted an antioxidant action in response to the inhibition of reactive oxygen species produced by neutrophils, this action would underpin its anti-inflammatory. The attenuation of the lesions and erythema due to acne rosacea could be the result of anti-inflammatory action and immunosuppressive drug.
